Of course there should be a journal on this topic! Open source medical software makes a lot of sense to me. I am a physician who has worked in the quality improvement field for the past 6 years working with hospitals and physicians trying to have an impact on the processes that occur in the office and inpatient setting. Hospital quality departments use free software; their budgets are so constrained that they often cannot consider much else. I would like to see more of an emphasis on quality improvement in the open source model since if we just replace paper records with electronic records we'll just replace a paper mess with an electronic one. The time has come to re-engineer the entire healthcare system so that it is more responsive to both patient and physician needs. There is no better way to do this than through computerization. The best way to guarantee voluntary adoption of the technology is to make it free. I'd be happy to be on an editorial board or just contribute. Charles Stimler
