Newcomers to the Free and Open Source Software (FOSS) in medicine scene have commented that it is difficult to discern which FOSS projects are the most advanced. At the risk of upsetting many worthy projects and hard-working people I bring the following short list of what I consider to be The Contenders: a United States centric view of those projects and resources that have achieved or are most likely to achieve a critical mass of users, developers and clinical ability. The criteria includes a Free license, a shipping product that is in actual use in real-world situations. Paying customers and availability of contractual support are highly desirable, but not completely necessary for the list. Apologies in advance to those that I have overlooked as well as a number of worthy International projects. Feel free to add to this list. Digg this article

2) TORCH http://www.openparadigms.com -- multi-specialty, ZOPE based.
3) SQLclinic http://www.sqlclinic.net -- multi-specialty.
4) VistA: http://www.hardhats.org, Medsphere/Vista http://www.medsphere.com
WorldVistA http://www.worldvista.org -- multi-specialty, supports large hospitals and large outpatient clinics, may work in smaller clinics.
Honorable mentions: tk_family practice/Medmapper, OIO.

FreeMED was noticably missing from the list. Though it is missing a downloadable tarball, it is quite stable, and is in use in several locations. (The lack of tarball is due to many fast changes in the API, which would have broken existing installations.) It is radically advanced from the last released version, and the FreeMED team is preparing a release of "0.6.0", which is currently in feature-freeze.
It has a highly advanced EMR, billing and scheduling systems, and a modular architecture, as well as XML-RPC services for connection with both GUI frontends and other programs. The released version will also be able to be HIPAA-compliant (according to the released HIPAA standards).
The homepage is http://freemedsoftware.com/, which should be coming online again in a day or so. (It was being redesigned.)
There is a workable demo available, as well as a commercial support infrastructure for FreeMED. People interested in the demo should contact jeff@ourexchange.net,
and people interested in the commercial support of FreeMED should contact irv@ourexchange.net.
There is also a variant based on an earlier version, called "FreeMED-YiRC", created by Joe Thielen. The homepage for that project is http://freemed-yirc.familyandyouth.org/, and features an EMR system designed specifically for youth in residential care settings.

They do not claim to be Free or Open Source on their Features page, so I presume that they have a proprietary license and are therefore a dead-end.

I already downloaded it for free here what they sell is the billing clearinghose services not the medical records system.
Though I first had to fix some date custom functions that worked using unix time instead of having no time frame restrictions I found it very easy to tweak and create forms is not complicated.
